Question about Cardiomyopathy

I have a question about cardiomyopathy. I believe treatment for symptom alleviation is prioritized when the patient has obvious symptoms like pleural effusion at the time of diagnosis. If the disease was found by chance or when the patient doesn't have any obvious symptoms (eg; when the patient just has loss of energy), what should I look at in order to determine the dose of medications? I usually perform x-rays, ultrasounds, and a non-invasive blood pressure measurement in order to determine the values of LA/AO, blood pressure, IVS, LVW, and LVIDd. However, I'm not sure about how far we should improve those values. If you have anything you use as an indicator when treating patients with cardiomyopathy, please let me know.
